The Sheet Metal Workers Local 91 Board of Trustees is pleased to offer you and your family access to excellent health care coverage. The Preferred Provider Organization (PPO) is administered by Blue Cross Blue Shield of Illinois and provides a network of hospitals and doctors across the United States. The Blue Cross Blue Shield network of doctors and hospitals have agreed to provide medical services to you and your eligible family members at discounted rates.

Enrollment Form

An Enrollment Form was sent to all Local Union 91 members and needs to be completed and returned as soon as possible. Failure to return may result in nonpayment of claims.

Privacy Statement

The Plans Privacy Notice

Effective April 14, 2004, the Health Insurance Portability and Accountability Act of 1996 (HIPAA), requires that the Plan protect the confidentiality of your private health information. A completed description of your rights under HIPAA can be found in the Plan's privacy notice that the Plan has distributed to you. You may obtain additional copies of the privacy notice from the Fund Administrator if you need another copy.

Under HIPAA, you have individual rights with respect to your protected health information, including certain rights to see and copy the information, receive an accounting of certain disclosures of the information and, under certain circumstances amend the information. You will also have the right to file a complaint with the Plan or the Secretary of the U.S. Department of Health and Human Services if you believe your rights under HIPAA have been violated.
